HOW DO I... REGISTER WITH THE PRACTICE?
All newly registered patients will
be asked to complete a questionnaire and undergo a new patient medical witht the docotr or nurse. Medical treatment is available from the date of registration.
HOW DO I... OBTAIN TELEPHONE ADVICE?
The doctors will give advice over
the telephone but you may be asked to call back if they are busy.
HOW DO I... MAKE AN APPOINTMENT?
Appointments can be made by telephoning
BOLTON 848411. We have four lines, which should improve the time taken
to answer your call. However, particularly in the mornings, all these lines are
busy and calls have to be redirected to "appointments", "prescriptions"
or the "office" (for test results and visit requests). Please be patient
if the telephone is not answered immediately - we will do our best to answer as
soon as possible. When you are placed on hold you will hear music and information.
You may consult any of the doctors
in the practice but it is advisable that you stay with one doctor for one particular
problem so that you can receive continuity of care.
Please note: we have early morning
surgery on Wednesday each week commencing 7.00am by appointment.
If you cannot keep your appointment
please let us know - it frees the appointment for somebody else.
Urgent Cases
To arrange an appointment please telephone (01204) 848411.
It is our aim that patients be seen by a health professional within 24 hours or a GP within 48 hours. If you wish to see a named GP then you may need to wait longer than this.
HOW DO I... OBTAIN A HOME VISIT?
If you are unable to get to the surgery
through ill health, a home visit can be arranged by telephoning 848411.
Except in an emergency, please telephone after 8.30 and before 10.30am. Please
be ready to give the receptionist your name, address, telephone number and a brief
description of what is wrong. You may be asked by the receptionist to speak to
the duty doctor.
HOW DO I... OBTAIN TEST RESULTS?
Results of investigations which have
been checked by the doctor may be obtained by telephoning 848411 after
2.00pm in the afternoon. Except in the case of children, results will only be
given to the patient concerned and not to relatives or friends.
